INTRODUCTION: During the coronavirus disease 2019 (COVID-19) pandemic, most healthcare workers (HCWs) were required to wear face masks for long periods of time. Since then, it has been shown that face masks have the potential to cause several physical adverse effects. This study aimed to estimate the prevalence of face mask-related complications among HCWs during the COVID-19 pandemic in Saudi Arabia. Furthermore, we compared the relationship between face mask usage with other variables, including the type of mask used and gender. METHODOLOGY: A cross-sectional study was conducted using convenience sampling, with a targeted sample of 517 participants (35% nonresponse). Data was collected via an electronic survey, the link for which was distributed through social media sites, such as WhatsApp and Twitter, to reach HCWs across Saudi Arabia. Data was analyzed using the SPSS software. RESULTS: Overall, 438 HCWs who wore N95 or surgical masks for 4 hours or more per day on average were recruited. Skin-related complications in the nasal area had the highest prevalence (342, 78.1%), followed by behind the ear area (333, 76.0%), cheeks (307, 70.1%), and chin (248, 56.6%). Other complications included headaches (226, 51.6%), and eye-related complications (211, 48.2%). All face mask-related complications, except for behind the ear skin complications, were more associated with female gender. CONCLUSIONS: Mask usage was significantly associated with the development of headaches, and eye, and skin-related complications. Female HCWs were more predisposed to these complications. Preventative measures and awareness activities should be considered to help reduce mask use related complications.

BACKGROUND: Factor XII deficiency can be related to either homozygous or compound heterozygous pathogenic variants in the F12 gene. The disease is commonly known as Hageman trait and is inherited in both autosomal recessive or dominant patterns. Clinically, factor XII deficiency is not associated with bleeding but conversely has been linked to thrombotic events, recurrent pregnancy loss, and hereditary angioedema. Molecular data of F12 deficiency are scarce and have revealed varying results between cases. However, most of the reported variants are missense mutations, gross deletions, or small insertion. Factor XII deficiency has been reported in the Saudi population in several studies, either as isolated case reports or included within the studies of rare bleeding factors deficiency. However, molecular data are lacking as no case report of genetic studies related to factor XII deficiency has been published in our local population, to the best of our knowledge. CASE REPORT: Herein we describe a homozygous missense variant involving exon 12 within F12 gene (5:176,830,269 G>A; p.Gly506Asp) in a 36-year-old Saudi multiparous female referred from the surgical clinic with significantly high activated partial thromboplastin time during preoperative assessment for sleeve gastrectomy. The patient had no history of bleeding episodes during the previous deliveries nor any tooth extractions. She had single event of spontaneous abortion during the 15th week of gestation without any bleeding complication. There was no history of thrombosis or skin manifestations, and she was not taking any medicines. There was no family history of bleeding or thrombosis. Family history revealed consanguinity as the parents are first-degree cousins. Physical examination was unremarkable. Upon investigation, the prolonged activated partial thromboplastin time was fully corrected by a 1:1 mixing study with normal pool plasma while lupus anticoagulant tests were negative. Factor assays and von Willebrand factor tests are all within normal ranges except for factor XII, which was severely deficient. A homozygous missense variant involving exon 12 within F12 gene (5:176,830,269 G>A; p.Gly506Asp) was identified. CONCLUSION: F12 (5:176,830,269 G>A; p.Gly506Asp) variant is likely to be a pathogenic variant among homozygous factor XII-deficient patients. Genetic counseling and management of the patients and families should be based on clinical evaluation.

AIM: To explore the specialty preferences and the factors influencing them among pre-clerkship (second-year and third-year) medical students at Alfaisal University-College of Medicine, Riyadh, Saudi Arabia. METHODS: An online, anonymous, cross-sectional, self-rating survey was administered. The survey explored socio-demographical data, specialty preferences and the factors influencing such preferences. A gender-wise statistical analysis was performed. RESULTS: Two hundred fifty-two students participated in the survey (n=252/308; response rate: 81.8%). The three main specialties chosen by males were general surgery (33.1%), pediatrics (7.9%), and neurology-ophthalmology (5.5%). Females also opted for general surgery (20.8%) followed by dermatology (11.2%) and pediatrics (8.8%). Gender-wise specialty preferences were noted: general surgery (p<0.028) and anesthesiology (p<0.045) by males, whereas obstetrics & gynecology (p<0.017) and dermatology (p<0.005) by females. Overall, the three major influences in choosing a specialty were "specialty interest" (86.5%), "specialty flexibility" (64.3%), and "anticipated income" (61.9%). Statistically significant differences were noticed between genders regarding the following factors: "specialty prestige" (p<0.005) by males and "culture-no opposite gender patients" by females (p<0.009). CONCLUSION: The overall two preferred specialties were general surgery (27%) and pediatrics (8.3%). Career counseling should be offered to students about each specialty's challenges/opportunities with an ultimate goal to match the country-specific demand and supply of physicians.
